Obtaining Medical Records

The Health Information Management department maintains records of the care patients receive while they are at Exceptional Community Hospital. As required by state and federal law, these records are kept in strict confidence and are only released with proper authorization from the patient.

To obtain your medical record, please visit the front desk at the hospital where you received your care and present valid identification. A copy of your medical record will be given to you.
